UAB Faculty and Staff with active Oracle appointments and all active UAB graduate students have access to IRAP automatically. The request form below is to be completed by current UAB Faculty or Staff to request access to the IRAP My Human Subjects (HS) module for individuals, such as undergraduate students, who are not active in IRAP.

Please note there is a separate process to obtain IRAP access for future UAB faculty and staff (see Incoming Faculty and Staff access to IRAP and LMS form).  

Procedure:

1. UAB employee (Faculty or Staff) completes and submits the Access to IRAP for IRB Submissions form below.  
2. IRAP office receives the Access to IRAP for IRB Submissions form and sets up access to IRAP.  This access will be Blazer ID and password for UAB students.
3. The IRAP office will notify the person who submitted the request and the person for whom access has been granted once the request has been processed.
