The Office of Medical Student Records maintains academic records for all students and alumni of UAB Heersink. Requests to obtain copies of various academic records and/or verification documents are made via the online Records Request System. You will use your BlazerID and password to login. If you do not have a BlazerID, you can get one by following the prompts on the portal home page.
Please note that fees may apply for some requests, and you may be redirected to a third-party site to enter credit card information (American Express not accepted).

Self-service letters and other requests (current students only):
Current medical students may also use the Records Request System to make updates to personal information and/or receive copies of some letters via self-service download within the portal system.
- Automobile insurance discount

- Name change request
- Name change requests should only be made through the UAB Heersink Records office, not through UAB OneStop.
- Deliver hard copy documents to the Records office in VH 107.
- Required documents: new social security card, marriage license/divorce decree/court documents/etc.
- Graduating MS4s: Diplomas are issued in the official name as recorded in the student portal. Diplomas are typically ordered 1 month prior to the diploma date; name change requests submitted after this date will incur a $25 re-order fee. No name change requests will be accepted after the diploma date.
- UAB OneCard: Get a replacement OneCard at any carding location on campus. Please allow at least 24-48 after UAB Heersink records office has completed the update. Replacement cards due to legal name change should be issued at no cost. Click here for more information.
- ERAS & NRMP: Check with Medical Student Services (MSS) for guidance on the timing and implications of a name change in ERAS and NRMP.
- Alabama Driver’s License: You must visit local probate or licensing office or visit an ALEA Driver License Office to be issued a duplicate license (fees may apply). You must present valid name change documents (e.g., marriage certificate, divorce decree, court order or legal name change document), along with proof that your name has been changed with the Social Security Administration. Graduating MS4 students who need documents for residency transition should click here for specific instructions on how to request documents leading up to graduation. Please note that the records office cannot release official documents until after the official diploma date.
