Lab Rotations
This form is to be completed prior to the start of a new lab rotation.
This form is to be completed by the student within 10 days of completing a lab rotation.
This form is to be completed by the mentor within 10 days of completing a lab rotation.
This form is to be completed by a student seeking to waive their 3rd rotation due to previous research experience and lab home selection.
This Adobe Sign form is to be completed by a student seeking a rotation beyond a fourth. This form must be completed prior to the start of the rotation.
This Adobe Sign form is to be completed by students changing labs and seeking another rotation. This form must be completed prior to the start of the rotation.
This form is to be completed by a faculty member who is currently accepting rotation students.

Mentor Selection Forms
This Adobe Sign form should be used when selecting a mentor.
This Adobe Sign form is to be completed by the student when selecting a new mentor. *With a change in lab, students will be expected to complete the Change of Graduate Study Committee form, seen below.

Graduate Committee
This AdobeSign form is used to nominate the mentor and members of a student's graduate study committee.
Change of the Graduate Study Committee can be made by submitting this AdobeSign form to the Graduate School for approval.
Committee Meetings are to be held once a year, at minimum. This form is to be completed within one week of the committee meeting.

Qualifying Exam & Admission to Candidacy
This form must be submitted two weeks prior to the scheduled QE to indicate the proposed written and oral QE dates.
Once the QE is complete, the student is responsible for completing the following AdobeSign form to be signed by each committee member.
This form is utilized for review between the student and committee.
This AdobeSign form must be completed and approved by the Graduate School before you can register for research hours. Please indicate the GBS office in the Program Manager field.

Dissertation Defense & Graduation
- Application for Graduate Degree/Certificate
Students planning to graduate in the upcoming term can access the Application for Graduate Degree/Certificate online through BlazerNET by clicking Links/Forms and selecting Apply for Graduate Degree/Certificate.
Submit no later than two weeks prior to your public defense so that handouts can be made and your defense advertised.
All doctoral students must submit a PDF of the final committee-approved dissertation for final review by the Graduate School.
This form is to be completed no later than 2 weeks before your dissertation defense.
This form must be submitted to the Graduate School with your completed dissertation.

Courses, Registration, & Grades
- Academic Appeal
Students can initiate an academic appeal under the Links and Forms section of their Student Profile in Blazernet. Once submitted, the appeal will be routed to the appropriate approver(s) (i.e. instructor, program director, GBS office staff, Graduate School) based on the type of appeal the student selected.
This form is utilized for course approval for 2-hour courses, off-campus courses, and non-GBS courses that require approval for advanced course credit.
This form is utilized for students that are requesting a course to be waived or substituted.
This Adobe Sign form is utilized for students in their final semester to request registering for below 9 hours.
This AdobeSign form is utilized for students who repeat a course taken at UAB and wish to request a grade replacement. This allows the first course to be excluded from their GPA calculation, once approved.

Joint Degree & Certificate Programs
This form must be completed before the coordinated courses are taken to indicate approval of the mentor and theme directors.

Other Forms
This AdobeSign form is utilized for students switching to another theme or program.
This AdobeSign form is utilized for students extending 7+ years in the program to adequately document their next steps towards degree and validate the expired coursework.
This Adobe Sign form is a student’s request to exit the program due to transfer, dismissal, or withdrawal. Prior to submitting the form, the student is required to meet with their theme director(s), GBS Program Director, GBS staff member, and (if applicable) their mentor to discuss the nature of the exit request. Contact the GBS Program Administrator for further information.
This Adobe Sign form is utilized for reimbursement of charges through GBS approved activities.
This form is to be completed for individuals requesting an additional position at UAB and submitted to the GBS Finance Manager.
This AdobeSign form is to be completed by students who wish to request a leave of absence.
This form is to be completed and submitted to the Graduate School Finance Director.
